2. I have read and understood that there are STEM OPT Reporting Requirements. link *

3. I understand that I need to submit required STEM OPT reports on ISS Website by each deadline. *

4. I understand that I need to submit a completed Form I-983 and follow the official instructions (link) in order to apply for STEM OPT extension to USCIS. *

5. Once STEM OPT extension is recommended and a new I-20 is issued, I must file Form I-765 with USCIS within 60 days of the I-20 issuance. *

6. USCIS must receive my Form I-765 application before my current post-completion OPT end date. I cannot file Form I-765 without a STEM OPT recommended I-20. *

7. I understand that I need to update any personal/employment changes within 10 days of the changes. *

8. If I change my employer, I need to obtain a new Form I-983 from a new employer and submit a STEM OPT Change of Employment Report on the ISS website and submit a final report for my previous employment. *

9. Federal law limits how long F-1 students on post-completion OPT or STEM OPT can be unemployed. The limits are:
• Post-completion OPT: 90 days
• STEM OPT: 150 days, which includes any days accrued while on post-completion OPT. *

10. When I have successfully completed my STEM OPT, I have a 60-day grace period to remain in the U.S unless I change my immigration status or transfer to another school. *

11. I understand if I file the STEM OPT extension application by the deadline and my post-completion OPT expires while the extension application is pending, USCIS will automatically extend my employment authorization for 180 days. This 180 day auto extension ceases once USCIS adjudicates my STEM OPT extension application.

12. I understand that ISS STEM OPT I-20 processing fee ($300) will be charged to my OSU bursar account when the I-20 is issued.
