Please use this form to set an appointment for in-person proctoring in the Office of Individual Study. The testing facility is located in Room 317 PIO Building, OSU-Stillwater campus. The I.S. office is open Monday-Friday 8:00 a.m. to 5:00 p.m. CST. Please know how much time is allotted for your exam and allow time to complete the exam at least 15 minutes before the I.S. office closes. A valid form of photo ID such as a student ID or driver’s license must be presented before you can take the exam.
Example: ANSI 3543, ENGL 4080 etc.
Example: Exam 1, Mid-Term Exam, Final Exam, etc.
Make sure that the requested exam date does not fall on an Oklahoma State University holiday.
Make sure that you allow for time to complete your exam. The exam must be completed by 4:45 pm CST.
