REQUEST FOR INTERN DS‐2019

CERTIFICATE OF ELIGIBILITY FOR EXCHANGE VISITOR (J‐1) STATUS

This form is to be used by OSU departments wishing to invite exchange visitors to OSU in the “Intern” category. The data and supporting documents requested with this form will allow International Students and Scholars (ISS) to complete a “Certificate of Eligibility for Exchange Visitor (J‐1) Status” commonly known as a DS‐2019 Form. The completed and signed DS‐2019 will allow the exchange visitor to apply for a J‐1 visa at a US Consulate/Embassy in their respective country. Please fully complete all sections of the form to ensure a faster processing time. Use Microsoft Edge as the browser.

The responsible OSU department hosting the Intern must provide ISS with the following:

Complete and sign the digital Intern DS‐2019 Request Form and the following required documents by selecting "Continue" button at the bottom of the page

1) Department Application:

A. Sponsoring OSU departmental letter of invitation describing terms and conditions and length of stay (should include financial support if appropriate).

B. J-1 English Proficiency Interview Assessment Report or TOEFL/IELTS Test Score

**Sponsoring Professor/Supervisor is responsible for conducting the interview and assessment, if the Intern does not have a TOEFL or IELTS score.

C. Training/Internship Placement Plan (DS‐7002)

You can find the DS-7002 file here (Note- We require this form to be completed in a digital format for immigration processing and upload it on page 9. After the processing of the DS-2019 is completed we will contact you for signature.)

2) Student Application:

**The following documents should be sent to the applicant/institution for completion and returned to the sponsoring OSU department. After the department receives all the completed forms, please upload them on last page. The file with the application can be found here

C. Home Institution Certification (Signature Required)

You can find Home Institution Certification template in page 5 of the form above.

D. Passport Picture Page Copies -Copies of biographical pages from passports of Intern applicant and all dependents

E. Financial Documentation for all sources ‐ letter from the Intern’s funding source(s) and letter of offer, bank statements, etc. All documents must be dated within 6 months of the request, have the exchange Intern’s name on the document, and specify amounts in US Dollars.

3) Prospective Intern applicant's digital Transcripts and resume/CV

4) DS‐2019s previous Copies- any previous copies of DS‐2019s and J-visas applicant may have held within past two years.

5) Transferring exchange visitor- Transferring Intern applicant from within the US, please provide copies of I‐94, visa, DS‐2019, etc.
