REQUEST FOR DS‐2019

CERTIFICATE OF ELIGIBILITY FOR EXCHANGE VISITOR (J‐1) STATUS

This form is to be used by OSU departments wishing to invite exchange visitors to OSU. The data and supporting documents requested with this form will allow International Students and Scholars (ISS) to complete a “Certificate of Eligibility for Exchange Visitor (J‐1) Status” commonly known as a DS‐2019 Form.  The completed and signed DS‐2019 will allow the exchange visitor to apply for a J‐1 visa at a US Consulate/Embassy in their respective country. Please fully complete all sections of the form to ensure a faster processing time. The responsible OSU department hosting the exchange visitor must:
   
1) Complete and sign the digital DS‐2019 Request Form by selecting "Continue" button at the bottom of the page

2) Along with DS-2019 request form, please include all the following required supplemental digital documents (upload on last page):

A. Sponsoring OSU departmental letter of invitation describing terms and conditions and length of stay (should include financial support if appropriate).

B. Prospective exchange visitor applicant's digital resume/CV

C. Financial Documentation for all sources ‐ letter from visitor’s funding source(s) and letter of offer, bank statements, etc.  All documents must be dated within 6 months of the request, have the exchange visitor’s name on the document, specify amounts in US Dollars. 

D. Exchange Visitor Applicant's Information

E. Passport Picture Page Copies -Copies of biographical pages from passports of exchange visitor applicant and all dependents

F. DS‐2019s previous Copies- any previous copies of DS‐2019s and J-visas applicant may have held within past two years.

G. Transferring exchange visitor- Transferring exchange visitor applicant from within the US, please provide copies of I‐94, visa, I‐20/DS‐2019, etc.

H. J-1 English Proficiency Interview Assessment Report or TOEFL/IELTS Test Score

**Sponsoring Professor/Supervisor is responsible for conducting the interview and assessment, if the exchange visitor does not have a TOEFL or IELTS score.
