Activity type*
Please choose the activity type that best describes your project. *
*Research-Basic: Experimental or theoretical work undertaken primarily to acquire new knowledge of the underlying foundations of phenomena and observable facts, without any particular application or use in view.
Research-Applied: Original investigation undertaken in order to acquire new knowledge. It is, however, directed primarily towards a specific, practical aim or objective.
Research-Developmental: Systematic work, drawing on knowledge gained from research and practical experience and producing additional knowledge, which is directed to producing new products or processes or to improving existing products or processes.
Instruction: Sponsored teaching or training work, or departmental research (e.g., curriculum development or evaluation) where participants are at least in part OSU students or employees.
Extension/Outreach: Programs and projects which involve the performance of work other than instruction and research. Extension is used to identify programs that are not research but programs where knowledge, expertise, and solutions are provided to the public as being supported by a grant, contract, or other award type. Outreach (also called public service) is used to specify activity supported by a grant, contract, or other award type where services are being provided to populations who might not otherwise have access to those services. Outreach has an educational role by raising awareness of existing services.
Equipment: Use when external funding is provided primarily for purchasing equipment.
Fellowship: Use when the funding opportunity is a graduate student fellowship where funding will primarily support the activity of a graduate student.
Training: Sponsored teaching or training work where participants are not OSU students or employees.
Other: Work does not fit any of the above categories, i.e. financial aid (funds from federal or state sources that are given to students to help pay for college; funds are typically given to students based on their financial need).

All senior/ key members of the research team must have a complete 1) Conflict of Interest disclosure on file with OSU, and 2) current (non-expired) CITI trainings as required by this project. CAS Research recommends research team members log into OneAegis and CITI to check these items and make any necessary updates immediately after submitting this form. If you have any questions, please contact the relevant University Research Compliance officer. *

Do any research personnel who will take part in the project participate in a foreign talent recruitment program (FTRP)**? *
**Based on the US Office of Science and Technology Policy’s National Security Presidential Memorandum 33 (NSPM-33), a foreign/international talent recruitment program (FTRP) is an effort organized, managed or funded by a foreign government, or a foreign government instrumentality or entity, to recruit science and technology professionals or students (regardless of citizenship or national origin, or whether having a full-time or part-time position). Some foreign government-sponsored talent recruitment programs operate with the intent to import or otherwise acquire from abroad, sometimes through illicit means, proprietary technology or software, unpublished data and methods, and intellectual property to further the military modernization goals and/or economic goals of a foreign government.
