Thank you for nominating a staff member for the Distinguished Service Award!
Anyone may nominate an OSU-STW staff member from any department or unit. While Staff Advisory Council members are not eligible for nomination, they may nominate other staff members. SAC Awards and Recognition Committee members are not eligible to submit nominations.
The nominee must be an OSU-STW staff employee who is employed at 0.75 FTE or greater and has not received a top 3 finalist Distinguished Service Award in the last 3 years.
To nominate a staff member, complete the nomination form below. The committee will consider letters of recommendations from OSU staff & faculty, a family member, and community members. Only one recommendation letter may come from a family member. You may provide one letter of recommendation via the recommendation form link below.
DEADLINE EXTENSION: Initial nominations must be received by October 10th, 2025 with recommendation letters submitted no later than October 17th, 2025.

You and the two people you list will receive an email with a link to the recommendation page. This page will have instructions and provide the questions you will need to answer about the staff member you have recommended. All questions should be answered fully for the staff member nominated to receive full consideration.
